D2740Replacement: 5–7 yrsPre-op radiographNarrative for structure loss

Crown — porcelain / ceramic note template

A complete, fill-in-the-blank D2740 note written around what insurance reviewers ask for — with the documentation checklist, the denial patterns, and an Open Dental auto-note format built in.

01 · Before you bill it

Know what the payer wants first.

The note should answer every item on the left — and read so that nothing on the right applies.

What documentation D2740 requires

Missing any of these is why the claim pends for records.
  • A structural indication in words: cusp fracture, failed large restoration, post-endo protection — not just "crown prep"
  • Pre-op radiograph attached; intraoral photo of the fracture is the strongest supplement
  • Quantified existing damage ("MOD amalgam exceeding 2/3 of the isthmus width")
  • Options-and-consent line showing alternatives were offered
  • A separate seat entry with cementation details

Why D2740 gets denied

The denial patterns payers actually use for this code.
  • Note describes decay a filling could fix — no structural necessity narrative
  • Missing pre-op film, or film taken after preparation (nothing left to prove)
  • Replacement crown inside the plan\u2019s 5-8 year replacement clause without documented failure

03 · Common questions

Frequently asked about D2740

Crown after root canal — automatic approval?
Posterior endo-treated teeth are commonly approved with the endo date and a film, but still write the structural line: access plus prior restoration leaves [X]% of coronal structure.
Same-day crown (CEREC) — anything different?
Same documentation; prep and seat can share a date of service. Keep the try-in/cementation details in the note.
