The note should answer every item on the left — and read so that nothing on the right applies.
What documentation D2740 requires
Missing any of these is why the claim pends for records.- A structural indication in words: cusp fracture, failed large restoration, post-endo protection — not just "crown prep"
- Pre-op radiograph attached; intraoral photo of the fracture is the strongest supplement
- Quantified existing damage ("MOD amalgam exceeding 2/3 of the isthmus width")
- Options-and-consent line showing alternatives were offered
- A separate seat entry with cementation details
Why D2740 gets denied
